Mrs. Lydia Hodge is the CEO of Faith Specialized Care Services Inc.
Mr. Clifford Smith is the Human Resources Director. His position includes the following duties: Interview and hire potential staff. Ensure that FSCS is compliant with the Department of Intellectual & Developmental Disabilities (DIDD) Provider’s Agreement hiring process which includes background checks and ensure that potential staff have the necessary documents needed to complete the hiring process. Once a staff is hired then they are set up for the following trainings if they do not have them at that time: CPI, Medication Administration of Unlicensed Personnel, Relias Training and First Aid/CPR. Once staff has completed training then they are to complete a Training to the Specific Needs of the Individual before they are to go to the homes to start their new shift. All Employee’s training and personal information is reviewed and filed in Human Resource Documents. Mr. Smith also ensure that payroll is approved and submitted to Paychex weekly. Mr. Smith is also the Corporate Representative Payee as well as Mrs. Lydia Hodge over the individual’s finances. Mr. Smith is the overseer of daily operations for FSCS.
Mrs. Patricia Hudson is the Administrative Assistant/Complaint Resolution Coordinator Her position includes the following duties: assist Human Resources Director with completing the hiring process by ensuring that the potential staff have current documents needed to proceed with the hiring process, verify employment and schedule employment interviews. She will maintain and schedule all medical appointments for the individuals. She ensures that all copies needed to run day to day operations are available, answer and direct calls to the appropriate people. She will ensure that all timesheets, mileage sheets and employment applications are filed. She ensures that all office supplies are ordered in a timely manner. Mrs. Hudson is the Complaint Resolution Coordinator for FSCS She oversees the complaints dealing with the Circle Of Support for the person’s served .
Mrs. Latanya Bailey is the Service Coordinator/Event Management Coordinator. Her position includes the following duties: Develop a Staffing Plan for the home. Supervise Home Managers and Direct Support Staff. Perform Unannounced Supervisory Visits in the homes. Ensure that all tracking documents is reviewed and filed monthly and as needed. Ensure that all medication, M.A.R. and Physician’s Orders are compliant for the individuals before they go to the homes. Ensure that Human Rights Restrictions are submitted and once approved filed in the individual’s records. Reconcile client’s finances and file them in their Financial Books. Perform home inspections to ensure that the homes are compliant with DIDD. Attend Circle of Support Meetings and ISP Meetings. As the Event Management Coordinator, she tracks and trends the Reportable Events and ensures that all reportable and non-reportable events are recorded and sent to DIDD and the appropriate entities.
Mrs. Marquitte Roberts-Cunningham is the Document Program Coordinator/Title VI Coordinator. Her position includes the following duties: Complete PCP/TN Care Billing bi-weekly. Ensure that all daily and overnight shift notes are reviewed and filed. Ensure that all ISP documentation which includes Level of Need Request, Provider Supporting Documentation, and Risk Tools are completed and filed in the Program books. Complete the Provider Monthly Review and submit to the ISC agency. Attend ISP Meetings and Circle Support Meetings. Review roommate match list and communicate with Individual Support Coordinators as well as DIDD Intake Representatives for potential clients along with CEO. Attend meet and greets with CEO for potential clients when the request for services is received. If a person has chosen FSCS to be their provider, we will work with the transition team to complete the transition. As Title VI Coordinator she will ensure that all Title VI forms are sent to the Person’s Supported as well as the Conservators, ensure that the person and their conservator understand their rights. She will ensure that they are signed at the ISP planning meeting. After they are signed, they will be filed with other records for the person’s supported.
